Affiliates Prime Plus was launched off of the success of Affiliates Prime and its parent company Affiliates in Physical Therapy, Inc to help provide more high-quality physical therapy care in the Houston area, particularly for seniors.

Affiliates in Physical Therapy, Inc. has been serving the Greater Houston area for over 30 years with a reputation for one-on-one care with high levels of experience and expertise. Affiliates Prime was developed as a unique brand for senior-focused and concierge care based on the model of Affiliates in Physical Therapy.

We are excited to expand the brand with Affiliates Prime Plus into the Tanglewood community!

  • Amy Gremillion

    PT, DPT, GCS, OCS, FAAOMPT

    Co-Founder/Lead Physical Therapist

    Amy graduated from Texas Woman’s University in Houston in 2013 with her Doctorate in Physical Therapy. After physical therapy school, she completed the Manual Therapy Institute Fellowship program in 2016 to become a Fellow of the American Academy of Orthopedic Manual Physical Therapists (FAAOMPT). She then became an Orthopedic Clinical Specialist (OCS) with the American Physical Therapy Association in 2017 and a Geriatric Clinical Specialist (GCS) in 2021. Amy has served on the Board for the Southeastern District of the Texas Physical Therapy Association since 2017 and has been an active member of the association since 2010. Amy has been a member of the Affiliates in Physical Therapy family since 2016 and helped kickstart the Affiliates Prime clinic in 2020.

    Prior to beginning her physical therapy education career, Amy attended Texas Tech University for her undergraduate degree in Exercise Science and was a member of the Texas Tech soccer team. She and her husband, Charlie, moved to Houston in 2010 and now have three sons. The family loves watching movies, playing sports, riding bikes, and spending time with their extended family in the Houston area.

  • Erich Folmer

    PTA

    Co-Founder/Physical Therapist Assistant

    Erich graduated from Texas Tech University with a Bachelor of Science Degree in Exercise Sport Sciences and went on to earn his Physical Therapist Assistant Degree from Coleman Health Sciences in 2014. Erich gained experience working in a number of different therapy clinics in Texas providing care for patients with various neurological and orthopedic injuries. Erich’s passion for learning and expanding his skills have led him to participate in many continuing education courses. He joined Affiliates in Physical Therapy in 2016 and helped kickstart the Affiliates Prime clinic in 2020.

    Erich takes great pleasure in spending time with his wife and kids. He has two daughters and one son. He enjoys participating in numerous outdoor activities including hiking, fishing, hunting, kayaking and camping.

  • John Cortez

    PTA

    Physical Therapist Assistant

    John is a Licensed Physical Therapist Assistant after graduating from the HCC PTA program in 1995. He has been practicing for 27 years and has extensive experience in geriatric physical therapy care. He has also worked in a variety of other settings including sports medicine, spinal cord injuries and closed brain injuries at TIRR, wound care, and aquatic therapy. He has been director and interim director at various clinics as well.

    When not at work, he enjoys spending time with his fiancé and his family, which includes six siblings, two daughters, two sons, and two grandchildren. He enjoys traveling, fishing, nature walks, music, and always dinner with a good movie.

    I worked with Erich and Amy on stretching and strengthening. I was having problems walking more than 3 miles without my leg buckling. They taught me great stretches and strengthening exercises that allowed me to go on a long-planned trip and successfully complete a road race in Italy. The emphasis on both stretching and strengthening helped fixed the problem. And they gave me advice on things that I could do (with compact equipment) while traveling to make sure I stayed in shape. This included researching exercise equipment I could take with me; that gave me a lot of confidence. I really appreciated their advice and care in getting me back into doing the distance walking I love to do.
